If you are a new patient, please contact your primary care provider and have your medical records faxed to us at 804-560-9577. If you are unable to find a desired time, please contact the office and the receptionist will be happy to accommodate you.

To reschedule an appointment, call us directly at 804-560-8880, option 1.
In order to expedite your visit, please bring the following items:
  • A list of current medications.
  • Insurance card(s) and a Photo ID.
New Patients, in order to finalize your registration process, please arrive 30 minutes before your scheduled appointment time. If you have internet access, you may go online to www.cardiologyrichmond.com to complete and print the new patient registration form(s).
Directions to the office are located on the website, or you may call the office for directions. 
If your insurance requires a referral to see a specialist, please contact your primary care physician's referral coordinator and request that it be faxed to us.
